


Welcome! Thank you for choosing Edgeworth Osteopathy. Please read the following information carefully as it forms an important part of your consultation and treatment agreement.
As an Osteopath I use my hands to diagnose and treat disorders of the body. As much as possible before and throughout treatment I will discuss my findings, treatment plan and any reasonably known risks with you.
Diagnosis and treatment will involve moving joints to test their range, and pressing on muscles to test for and
treat restrictions. Injured areas may be painful to touch or move and it is often necessary to treat connected areas to reduce restriction to the painful area. For example if you have a sore lower back, it is likely that the hips and upper back may also be affected.
If you have any concerns about any area of your body or areas that you
don’t want treated (e.g. areas culturally sacred) please let me know at the start of your session.
If you have any preferred styles of treatment, or techniques you don’t like (e.g. joint manipulation), please let me know at the start of your session.
In order to fill a cancelled appointment we require at least24 hours notification. Less notice than this may attract a late cancellation fee of up to 100% of the appointment fee. Missed appointments will incur a 100% cancellation fee, as someone could have come in your place.
We understand that sometimes the unexpected can occur and will endeavour to be understanding to your situation and circumstances. In such cases please contact us as quickly as possible. Cancellation fees may be waived at the discretion of Edgeworth Osteopathy.
In cases of contagious illness (flu etc.) please call as soon as possible to reschedule. You will not be charged in such cases.
Should I need to cancel your appointment I will hold myself to the same terms and conditions as above, and provide at least 24 hours notice, or your next appointment will be free.
Your phone number and/or email address will be used to send an appointment reminder and confirmation
(usually the day before your treatment). These reminders are a courtesy and the primary responsibility for remembering your appointment remains with you. In addition, your details will be used to contact you should it become necessary to change your appointment time, request permission to release your information to a third party (e.g. your doctor), inform you of test results or scans or inform you of any other relevant information that cannot wait until your next appointment.
Your phone number and/or email address may also be used for marketing and promotion purposes by Edgeworth
Osteopathy. These details will not be shared with any third party (other than Cliniko where they will be stored). You

Your emergency contact person and their details will be used in the case of a serious medical incident or emergency to inform them of your location, state of health and/or safety.
All information provided is held in the strictest confidence and will not be shared with any third party without
your permission unless required by law. All information from your case history form and signed consent forms will be transferred onto an online Australian based software program (Cliniko) which meets Australian privacy law
obligations. Paper forms will be stored securely or shredded.
